We at DBAM Social Care enable discharges from hospital through integrated, multi–agency working and joined-up service provision for individuals with ongoing health and care needs. Our Hospital Discharge service is designed to help people to keep on recovering and be rehabilitated within their home and community environments.
What is Hospital Discharge Care
Hospital Discharge is a service that assists individuals discharging from hospital into the home so that they can carry on their recovery in the appropriate manner.
It includes re‐ablement services that allow people to be restored to their baseline, so they can remain independent and in charge of their lives. This service can be a simple discharge—with little or no additional care—to a more complex discharge, where a short‐term package of bed‐based or home‐based care and recovery support is provided subject to further evaluation of longer‐term care needs.

DBAM provides targeted care for dementia and Alzheimer’s patients, ensuring maximum quality of life through individual care plans that increase comfort and safety. Our Hospital Discharge allows for a seamless hospital to home adjustment, with continued recovery supported through tailored care.
Physical Disability Care
DBAM offers specialist care for physically disabled individuals, with daily living and mobility support to allow clients to be as independent as they can in their own homes. Our Hospital Discharge ensures patients are provided with proper re‐ablement and support at discharge from hospital and back to their community.
Stroke Care
Aftercare for stroke is tailored to the needs of recovery of the individual, with rehabilitation and support to gain independence in everyday life. Hospital Discharge is part of this, and it offers bespoke recovery packages that support the patient to make safe hospital-to-home transfers.
Multiple Sclerosis Care
Care for multiple sclerosis clients involves symptom management, functional support and provision of a home that is comfortable and suited to their requirements. Our Hospital Discharge service ensures co‐ordinated care to assist clients in managing their symptoms as well as in pursuing further rehabilitation in the comfort of their own homes.
Spinal Injury/Autonomic Dysreflexia (AD) Care
Specialist care is available for patients with spinal injury or autonomic dysreflexia, focused on safety, mobility support, and management of specific health issues. In addition, our Hospital Discharge service offers targeted re‐ablement intervention to assist such individuals to become independent after discharge from the hospital.
Mental Health Conditions
DBAM provides compassionate care for mental health clients, with a safe environment and emotional support to promote well‐being and recovery. Our Hospital Discharge service is planned to ensure a smooth transition from hospital treatment, with continuity of mental health care.
Parkinson's Care
DBAM delivers specialist care to people with Parkinson’s disease, coordinating their particular needs through responsive support and mobilization and living assistance. Hospital Discharge service is the linchpin in the care pathway for these individuals, emphasizing the structured method to support and re‐ablement after hospital discharge.
